Reporting an incident

Document AED deployments and other equipment uses while details are fresh.

When equipment is used in a real event, report it under Incidents while details are fresh. Inspectors and above can report.

An AED incident captures#

  • Whether the AED was used, whether a shock was advised, and how many shocks were delivered.
  • Whether CPR was performed, when EMS was called, and when they arrived.
  • The outcome, the responder involved, and a free-text narrative of what happened.

Other equipment types have their own tailored reports — a fire-extinguisher discharge asks about fire class and whether it was extinguished; a naloxone administration asks about doses and response. The form fits the event.

Good practice#

  1. Report the same day — narratives written a week later are vague.
  2. Attach photos and documents (up to 10 MB each): scene photos, EMS paperwork.
  3. Do the housekeeping: an AED that delivered a shock almost always needs new pads — update its consumables, and record a post-use inspection.

Note: Patient details are a separate, access-controlled section — see Patient information and privacy.
